He was predeceased by: his parents, James Wallace Polk and Mae Louise Polk; and his brother James E. Polk (Ellen). He is survived by: his wife Margaret; his children, Jennifer Robins (Christian), Kristin Viox (Matthew) and John (Caroline); and his grandchildren, Evie, Cash, Gracie, Layla, Luke, Tommy and Judd.
In lieu of other expressions of sympathy donations are requested to the St. Jude Children's Research Hospital.
