He is survived by: his children, Laurie Plate (Scott) of Atlanta, GA and Jeff Wiedman (Lisa) of La Crosse; his grandchildren, Matthew Plate (Kiley), Rachel Fundora (Pepe) of Atlanta, GA and Abby Warren (Lucas) of La Crosse; and his great grandchild Charlee Mae.
In lieu of flowers, the family asks that memorials be directed to Gundersen Medical Foundation for the Gundersen Hospice Program, which cared so well for Jerry in his final days.
