He was predeceased by: his parents, Michael R. Cholak Sr. and Patricia Cholak (Hase); and his brother Ernie Heal. He is survived by: his wife Julie Kuryanowicz; his children, Rachael Teagan (Mikey Usher) and Evan Michael (Madison Earing); his in-laws, Ron and Janet Kuryanowicz; his siblings, Terry Heal, Kathy Jefferson (Mike) and Joe; his sister-in-law Sue Kuryanowicz; his nieces, Morgan, Peyton, Patty and Peggy; his nephew Carter; and his close friend Kristine Coshun (Dicky).
Visitation was held on Friday, November 15th 2024 from 12:30 PM to 4:30 PM at the Moose Club (3003 30th Ave, Kenosha, WI 53144). A funeral service was held on Friday, November 15th 2024 at 3:00 PM at the same location.
