She is survived by: her mother Joyce Hazen; her husband Dave Johnson; her children, Jeffrey Hazen (Jodi Shea), Richard Zehe and Rachael Prochnow (Jason); her stepchildren, Christopher Johnson and Brooke Hady (Christopher); her grandchildren, Joselyn Watson (Kyle Casper), Alethia Hazen (Austin Blaisdell), Jeffrey Hazen, Ricky Zehe, Jasmine Sylvester, Karmah Zehe, Essa Zehe, Sydney Prochnow, Addison Prochnow and Grady Prochnow; her stepgrandson Nolan Hady; her siblings, Gregory Hazen (Nicola), Kevin Hazen (Delores), Anthony Hazen, Connie St. Martin, Christine Jordan and Angela Hazen; the father of her children Richard Zehe; and her father-in-law Al Johnson. She is also survived by many aunts, uncles, nieces, nephews, and cousins.
