He was predeceased by: his parents, Lester Onsgard and Ruth Onsgard (Turner). He is survived by: his sons, Jeremy Onsgard of Gillette, WY and Joshua Onsgard (Kate) of Grand Marsh, WI; his grandchildren, Faith, Ethan, Hadlie, Ewan and Juniper; and his brother Jere Onsgard. He is also survived by other extended family and friends.

Memorials may be made in Jack's name to Parkinsons Foundation, 200 SE 1st Street, Suite 800, Miami, FL 33131.
