He is survived by: his wife Carol Fornal; his children, Kendall Welborn (Cindy) of Crystal Lake, IL, Colleen Prifogle (Ben) of Oconomowoc, Mike Fornal (Laura) of Mequon and Mark Fornal (Amber) of Racine; his grandchildren, Morgan, Max Prifogle, Nick and Corinne Fornal; and his siblings, Ginger, Steve and Gina. He is also survived by nieces, nephews other relatives and friends.
