He was predeceased by: his parents, George Heinzelman and Arlene Heinzelman (Peterson); and his brother Jim. He is survived by: his wife Susan; his son Jay; his grandchildren, Charlie and Jason; his sister-in-law Judy Griffin (Bob); and his niece Kim Steffes.
In lieu of flowers, memorials in Jack's name may be made to the Badger History Group, UW Health Carbone Cancer Center, or the American Cancer Society.
