He was predeceased by: his parents, Catherine H. Sullivan and Patrick J. Sullivan; his daughter Catherine Sullivan; his siblings, William, Jerry, John, Edward Sullivan and Patricia Conditt; and his siblings-in-law, Kenneth, Gary, Harold "Bibs" Evenstad and Carol Kinney.

He is survived by: his former wife Jacqueline M. Evenstad; his wife Jackie; his children, Michelle Cordeiro (Robert Hein) of Neosho, WI, Barbara Summers (Rick) of Mesa, AZ, Sean Sullivan (Emily) of Columbus, WI, Brian Sullivan (Carla) of Saudi Arabia and Jackie Blank (Mark) of Watertown, WI; his grandchildren, Mandi Bruce (Jarrett), Rex Holtz (Lisa), Courtney Jeanpierre (Jordan), Tanner, Gaven Hein, Nick, Shanna, Kayla, Marcelo Isa, Sebastian, Kiara, Sean Sullivan, Kaitlyn, Tamara Sell, Lindsay Blank and Zach Sell (Emily); his siblings, Ellen and Thomas Sullivan; and his siblings-in-law, Micki Sullivan, Marvin Krogerson, Rick Krogerson, Maryann Fischer and Peggy James. He is also survived by relatives and friends.
