She was predeceased by: her parents, Glenn Polk and Ruth Polk (Piquette); her sisters, Peggy Hill (Craig) and Theresa Steib; and her niece Bridget Polk.

She is survived by: her husband Bruce; her siblings, Mary Witte (John), Madison, John Polk (Nancy), Judy Weimer (Gary) of Lombard, IL, Jim Polk (Sue) and George Polk; and her sister-in-law Karen Durnin (Tim) of Melbourne. She is also survived by 16 nieces and nephews.

Memorial donations can be made to Holy Hill Bascilcia in Anne's name.
