She was predeceased by: her husband Myron Smith (Mike); her daughter Leslie Smith; her parents, Albert Malone and Jennie Malone; and her brothers, Dr. John Malone and Lawerence Malone (Audrey).
She is survived by: her daughters, Shari Smith-Mead (Marshall) and Lori Futh (Richard); her granddaughter Rachel Miller (Rick); her great grandchildren, Xander and Alexis; and her sister-in-law Kristie Malone. She is also survived by nieces, nephews, cousins, other family and friends.
In lieu of flowers donations to Shriners Children's Hospital, the Heart Association of Wisconsin or the Veterans of Foreign Wars in Carloyn's name are appreciated.
