He was predeceased by: his parents, Jeannette Petersen and Gerald Kurth. He is survived by: his spouse Jan Levi; his grandchildren, Logan, Sydney, Kloe, Jayce, Mac, Easton, Matt, Cheyne and Megan; his sister Sharon Jensen (Neil); and his pet Annie. He is also survived by several nieces and nephews, many close friends.
