He is survived by: his children, Jodi Walter (Charles), Tracey Boehm (Chris), Salem Kazik, Quade Kazik and Delain Kazik; his grandchildren, Candace Hines (Daniel), Christopher and Leah Vannieuwenhoven; his great grandchild Brantley William Hines; and his sister Sharon Freeman. He is also survived by nephews, other relatives and friends.
