He was predeceased by: his father Gerald Cook. He is survived by: his mother Dorthey Cook (Roller) of Granton; his wife Jeanie Long of Tomahawk; his daughters, Kayleigh Conlin (Cory) of Houston, TX and Tasha Cedeno of Middleton; his stepdaughter Emily Rand (Jordan) of Littleton, CO; his siblings, Jeff Cook (Denise) of Merrillan, WI and Alisa Eichhorn (Chris) of West Bend; and his grandchildren, Connor, Paige, Delaney, Anniston, Ila, Bridger and Carter. He is also survived by his 6 nieces and nephews.
