He was predeceased by: his parents, Neville Cook and Thelma Cook (Weaver); his son Jeff Cook; and his siblings, Robert Cook, Kenny Cook and Arlene Wiese (Cook).
He is survived by: his children, Keith Cook (Jill), Brenda Cook (Kirk), Deanna Cook and Kim Cook (Ryan); his close friend Theresa Bender; and his brother V. Ray.
