She was predeceased by: her parents, Raymond Nowak and Bess Nowak; her husband James Kozlowski Sr.; her brother James Nowak (Janice); and her daughter-in-law Heather Kozlowski.

She is survived by: her children, Joel Kozlowski, Ashley Mack (Ryan) and Rachel Kozlowski; her nieces, Lori Shaneck (Steve), Pamela Maki and Vicky Nowak (Dan Lepkowski); her pets, Lucky and Señor; and also Linda Krause. She is also survived by children and grandchildren.

If desired, memorials to the Wisconsin Humane Society, St. Camillus Foundation or the charity of your choice are appreciated.
