He was predeceased by: his parents, Juan Alvarado and Juliana Alvarado Sr. (Estevis); his son Christopher Alvarado; his brothers, Hector Alvarado and Raul Alvarado; his brother-in-law Raymundo Montemayor; his nephews, Roberto Montemayor and Guadalupe Alvarado; and his niece Holly Alvarado.

He is survived by: his children, Jason Alvarado, Joshua Alvarado and Cassandra Alvarado; his siblings, Juan Alvarado, Jr. (Vicki) of Whitewater, WI, Jose Alvarado (Patti) of Mount Horeb, WI, Gilbert Alvarado (Lynnette) of Fort Atkinson, WI, Albert Alvarado of Richville, IN, Thomas Alvarado of Fort Atkinson, WI, Gary Alvarado of Fort Atkinson, WI, Donald Alvarado of Fort Atkinson, WI, Zulema Montemayor of Webberville, MI, Margaretta Sanchez (Andrés) of Janesville, WI and Carolina Mayans (Reynaldo) of Fort Atkinson, WI; and his sister-in-law Joanie Alvarado of Fort Atkinson, WI.. He is also survived by his step-children, grandchildren, many nieces and nephews, and several close friends.
