He was predeceased by: his parents, David Haack and Ruth Agnes Haack; his sisters, Darlene Haack Racinowski and Patricia Haack Gratton; and his son Michael David Haack.

He is survived by: his wife Mary Ellen Haack; his children, Dawn, Brian Kennedy, Tim and Molly Haack; his grandchildren, Ryan Kreklow, Gabrielle Kennedy, Amanda, Garth Schumacher, Jillian, Darin Tuttle and Cody Stafford; and his great grandchildren, Kane, Elaina Schumacher, Lucy Schumacher, Hudson Schumacher, Lydia, Lincoln and Lennon Tuttle.

In lieu of flowers, memorials can be directed in David's name to St. Jude's Hospital or Critter Junction Pet Rescue/Fond du Lac.
