He is survived by: his parents, Bradley Church and Robin Milbrandt of Evansville; his sisters, Breanna Cottrell (William Cottrell) and Rochelle Church; his nieces, Savannah Cottrell and Hallie Cottrell; his grandparents, Linda Church (Gary Topp), Peggy Wayne, Anthony Wickersham and Mike Asmus; his uncles, Bret Church (Sheri Biddick) and Richard Wayne of Janesville; his aunt Bobbie Jo Wayne of Janesville, WI.; his great-aunt Ann Nordness (Dennis); his great-uncle Steve Flynn; his grandmother Lois Peterson; his great-grandparent Bertha Milbrandt; his grandfather Keith Church; his uncles, Dave Milbrandt and Thomas Porter; his aunt Terri Wickersham; and his cousin Marya R O'Dierno.
