He is survived by: his wife Jody Folstad; his daughters, Emily Folstad (Drake Friesen) of Minneapolis, MN and Jenna Folstad of Madison; his stepdaughters, Jamie Shotliff of Evansville and Megan Bergum (Michael Domke) of Evansville; his children, Paige, Daniel, Sidney, Braiden, Brogan and Gracie; his brothers, Brad Folstad of Evansville and Brent Folstad (Jackie) of Footville; and his nieces and nephews, Amanda Olson (Bob), Andrea Matthews (Brian), Cassie Tomczak and Chad Folstad.
