He was predeceased by: his wife Shari; his parents, Rose Cook and Howard Cook; his sister Colleen Zehfus; his sons, Thomas Cook and Stephen Cook; and his grandson Joey.
He is survived by: his daughter Patricia Schmitt (Joseph); his grandchildren, Amy Cook (Cameron Kurth), Elizabeth Berndt (late Matthew), Katherine Nobui (Kyle), Lauren Cook and Anthony Schmitt (Katherine); his stepgrandchildren, Josh Williams (Alicia) and Jacob Williams; his great grandchildren, Owen Stackhouse, Shawn Schmitt and Henry Schmitt; his stepgreat-grandchildren, Megan Williams, Katelyn Williams and Jayden Williams; and his daughters-in-law, Diane Hegwood and Patricia Dunn.
In lieu of flowers, donations may be made to the Humane Animal Welfare Society of Waukesha or to the Northwood Wildlife Center in Minocqua.
