She was predeceased by: her parents, Louis Geurts and Stella Geurts (Van Duyse); her husband Mel Hazaert; her sisters, Bernice Tarkowski and Mary Ann Geurts; and her great grandchildren, Amelia and Josephine Vander Heyden.
She is survived by: her children, Paul Hazaert (Karen) and Pat Rhodes (Dale); her grandchildren, Dusty Rhodes, Trisha Vander Heyden (Scott), Derek Rhodes (Amanda), Drew Rhodes, Matt Hazaert and Sarah Hazaert; her great grandchildren, Bailey, Kendra, Emmit Hazaert, Cooper, Eleanor, Graham, Emmaline Vander Heyden, Addison, Dominic and Finley Rhodes; and her brothers, John Geurts (Pat) of De Pere, Dick Geurts (Mary) of Greenleaf and Ralph Geurts (Janice) of Oshkosh.
