He was predeceased by: his parents, Earl Smith and Elizabeth "Betty" Smith (Archiquette); and his brothers, Rev. Earl Smith and Bradley Smith. He is survived by: his wife Joan Rose; his daughters, Brenda Haen (Robert), Karen Smith and Cynthia Smith; his grandchildren, Zachary and Brianna Haen; and his siblings, Karen Skenandore, Judith Matthews, Stanley Smith (Linda), Gary Smith (Cheryl), Daryl Smith, Elizabeth Kramer (Rick), Susan Loch (Matt), Larry Smith (Donna), Chrystal Smith and Sarah Smith (Heidi Smith). He is also survived by extended family and many friends.

In lieu of flowers, a memorial fund will be established in honor of Lew.
