He is survived by: his sons, John and Michael; his wife Diana Sullivan (Meyer); his children, JT Sullivan of Mount Horeb and Kelly Sullivan (Alden Broome) of Waupun, Wis.; his brother Ray Sullivan of Mount Horeb, Wis.; his niece Tricia Ayers (Bob) of Lake Elmo, Minn.; and his pet Toby. He is also survived by many nieces and nephews.
