He was predeceased by: his parents, Paul E. Mathis and Ruth Ann Mathis (Gamrath); his brother Steven; and his siblings-in-law, Charles, Elaine VanderZanden and Jedd.

He is survived by: his wife Pat; his stepdaughters, Melissa Strait (Chad) and Jodi White (Craig); his grandchildren, Zach, Caden, Katlyn, Brenden and Kendryn; his brother Gary Mathis (Janice); his sister-in-law Leslie Mathis; his niece Danielle Mathis; his nephew Matthew Mathis; and his sisters-in-law, Judy Ziegler, Kay Ponschock (Randy) and Nora Hermus (Ken).
