He was predeceased by: his parents, Clyde "Leroy" Slade and Grace Slade; and his daughter Lindsey. He is survived by: his significant other Gloria Slade of Columbus; his daughters, Lea Henrichs (J. R.) of Columbus and Becky Pederson (Brian) of Boerne, TX; his grandchildren, Brandon (Zoe), James (Kara), Emily, Gavin and Zade; and his sister Denice Kachelski (Ken). He is also survived by seven great-grandchildren.
