He is survived by: his spouse Vivian Pake; his children, Beth Hill (Gary) of Bloomer, Jeffrey Wallace (Nanette) of Colfax, Brian Wallace of Sand Creek and Penny Alman (Loren Prince) of Bloomer; and his brother John "Jack" Cizek of De Moines. He is also survived by 11 grandchildren and 20 great grandchildren; numerous nieces nephews, other relatives and friends.
