He was predeceased by: his parents, Charles Hubka and Flora Hubka; his brother Roger; and his sister-in-law Shelia Salisbury. He is survived by: his wife Janet Salisbury; his sons, Michael and Mark (Theresa); his granddaughter Emily Hubka; and his brother-in-law Richard Salisbury.

Memorials would be appreciated to Beloit Regional Hospice.
