He was predeceased by: his mother Deborah Hazelton (Thibedeau); his grandparents, Phillip, Elizabeth Hazelton, Gladys and Roy Thibedeau; his aunts, Paula Kvapil and Donna Thibedeau; and his cousin Chris Thibedeau.
He is survived by: his father Rick Hazelton (Patty Reedy) of Chippewa Falls; his significant other Elizabeth Roshell of Chippewa Falls; his children, Arlyona Hazelton and Domenic Hazelton; his pet Zeus; his siblings, Tamra Fabian (Ken) of Madison and Richard Hazelton (Kelly) of Chippewa Falls; and his nieces and nephews, Eric Fabian (Ceci), Alex Fabian (Chelsea), Logan Liddell, Autumn Pawlak and Ashtyn Hazelton.
