He is survived by: his children, Andrew Kubista and Brenda Kubista (Jerrimiah Millard); his grandchildren, Lorelei Kubista and Dustin Millard; and his siblings, Jerry Kubista (Betty Rogers) and Jean Kubista. He is also survived by many other relatives & friends.
