He was predeceased by: his parents, Robert Hayes and Maxine Hayes; his siblings, Mark Hayes and Barbara Peterson; and his brother-in-law Richard Peterson.
He is survived by: his niece Caitlyn Dunphy (Shawn); his nephew Jeff Allen; his great-nieces, Due and Isabella Dunphy; and his friends, Carol Lehsten and Julie.
In lieu of flowers, memorial contributions may be made to Aurora Medical Center in Burlington.
