He was predeceased by: his parents, Henry and Katherine; his siblings, Henry, Carl and Katherine; his stepson Jack Van Sistine; and his former wife Gloria.
He is survived by: his wife Gwen; his daughters, Sheena (E. K.) and Billie Joe (Adam); his grandchildren, Eric, Kellie, Addie and Kinley; his son Lee (Heather); his grandsons, Quinton and Bob Hoffmann (Amy Lee); his sons, Tom Hoffmann (Pam) and Jamie (Maura); his grandchildren, Makenzie and Evan; his daughters, Amanda (Kyle) and Cindy Johnson (Kevin); his grandchildren, Kaylin and Desmond; his brother Bob Hoffmann (Lois); and his stepchildren, Bob, Harold, Annie Van Sistine and Lynn Leuquin. He is also survived by several nieces and nephews.
In lieu of flowers, Dan's family kindly requests that you raise a glass, preferably a vodka martini—straight up extra dry, with olives— and tell a joke in his honor.
