He was predeceased by: his parents, Paul J. Kujak and Martha Kujak (Bagnewski); his wife Donna Lou Danielson; his daughter-in-law Shari; and his siblings, Ray, Helen, Ethel, Ilene and Dorothy.
He is survived by: his children, Patty Hough of Rhode Island, Paul Kujak of Baraboo and Perry Kujak (Renee) of Blair; and his sister Betty Jean Hentges (Ed) of Fountain City. He is also survived by thirteen grandchildren, five great-grandchildren.
