He is survived by: his mother Cheryl Cook (Ed Holmstrom); his wife Kim Swartz; his children, Adam, Levi Swartz and Amanda Jo Swartz; his sister Carri Jo Bunker; his nieces and nephews, Tyler Bunker and Casondra Jo Shaffer; and his cousin Jimmy Peppin (Loren Purintun). He is also survived by many aunts, uncles, other relatives & friends.
