He was predeceased by: his former wife Kathleen Weiner; his parents, Catherine Kujawa and Joseph Kujawa; his sister Dorothy Trzesniewski (late Ray); his in-laws, Raymond, Emma Weiner, Harry and Molly Eldred; and his siblings-in-law, Roger, Lucy Weiner, Jerry, Joyce Weiner, Robert and Monica Anderson.
He is survived by: his wife Sue Eldred-Kujawa; his children, Patrick Kujawa (Lenore), Peter Kujawa (Stephanie) and Colleen Boraca (Carl); his grandchildren, Taylor, Brennan, Karina, Ryan, Emma, Henry, Maggie and Theo; his stepdaughters, Rachel and Meg Cychosz; his sister Nancy Aschenbrenner (late Jerry); and his siblings-in-law, Phil Weiner (Nancy), Roxanne Weiner (late Robert), Bill Eldred (Tracy), Jim Eldred and Janet Eldred. He is also survived by nieces, nephews, relatives and many close friends.
In lieu of flowers, memorial contributions may be sent to the family for the Dr. Anthony J. Kujawa Memorial Scholarship established at Baraboo Community Scholarship Corporation.
