Worthen (Jacob); his siblings, Justin, Josie Worthen, Austin Dolderer, Tucker Johnston and Blaine Gloeckler; his grandparents, Bill, Bernadette Johnston, Carol Sorensen, Jim, Mary Maxwell, Phil, Shelia Worthen and Sandra Worthen; his great-grandparent Elizabeth Johnston; and his pet Nacho. He is also survived by aunts, uncles, cousins other relatives and many special friends.
