He was predeceased by: his grandparents, Bill, Virginia Duncan, Tom Lewis and Beulah Tackett; and his brother Gerald Lewis.

He is survived by: his parents, Harold Lewis and Diane Lewis (Brian); and his siblings, Tammy Stone (Doug), Melissa "Missy" Baker (Tim), Trisha Kloet (Jeff), Kim Lewis, Harold Lewis (Laura) and Terry Lewis (Lori). He is also survived by many aunts, uncles, nieces, and nephews.
