He was predeceased by: his parents, Heinz and Eleanor (Whoelert); and his sisters, Nancy Hellman of Johnson City, TN and Ann Wagner (Dale) of Hartford, WI.. He is survived by: his wife Shirley; his children, Kerry (Colleen) of St. Augusta, MN, Lindsey Monroe (Ryan), Regan, Reed, Justin (Holli), Hayden and Shelley Meyer (Jim) of Deerbrook; his grandchildren, Josh and Haley; his son Jon; his grandchildren, Brendan and Alek; his great grandchildren, Ross and Kora; and his brother Heinz Ewert (Becky) of Portage, WI.

In lieu of flowers, a memorial will be established in Jack's name.
