He was predeceased by: his grandparents, Berlie Moore, Walter and Lillian Huseby. He is survived by: his parents, Gary Moore and Karen Moore; his half-brother Joshua Moore (Lanccia); his grandmother Joyce Moore; his pet Synnova; and his friends, Riley (Lauren), Jake (Virginia), Josh (Shelby), Tyler and Bridget. He is also survived by many family members and friends.

In lieu of flowers, the family requests that memorials be made in Jacob's name to the American Foundation for Suicide Prevention (AFSP) or any organization dedicated to suicide prevention and supporting survivors.
