He was predeceased by: his parents, Arlene Hazelton (Stafford) and Edward Hazelton; his stepfather Lawrence Hazelton; his daughter Traci Hazelton; his brother Edward Hazelton; and his brother-in-law Wayne Bollom.
He is survived by: his former wife Terri Knutson; his children, Staci Bowe (Red) of Eau Claire and Steven Hazelton (Tabytha); his grandchildren, Mya, Sa'Mariyah, Martina and Ja'Liya of Eau Claire; and his sisters, Shirley Peck (Roger) of Jim Falls, Pam Bollom of Chippewa Falls and Sharon Clouse of Spring, Texas.
