He was predeceased by: his parents, George Kuba and Eunice Kuba (Hendricks); his foster parents, Jack Shaver and Charlotte Shaver (Wendel); his siblings, David Henricks, Richard Kuba and Cindy McCutchen; his foster brother Johnny Shaver; his brother-in-law John Abler; and his parents-in-law, Isabelle, Walter Oakley, Lonnie and Janice Pease.
He is survived by: his wife Donna Jean Oakley; his close friend Holly Manthey; his former wife Donna Kuba of Chilton; his children, Christopher Kuba of Green Bay and Jamie Kuba Gjellstad (Clint) of Velva, ND; his grandchildren, Aubrey Kuba, Charlotte Kuba, Odin Gjellstad, Isabelle Gjellstad, Ella Schuenke, Christian Manthey, Sammy Manthey, Ethan Stitely, Alaina Stitely and Gemma Manthey; and his siblings, Mark Kuba (Jean), Sharon Lawver, Melinda Kraft, Bonnie Abler, Chuck Shaver (Debbie), MaryJo Mullenbach (Joe), Penny Maleski, Scott Pease (Becky) and Rick Pease.
As an expression of sympathy, memorial contributions may be sent to the family to help with funeral expenses.
