He is survived by: his father Tom Kozlowski; his children, Jesse Konen, Josh Kozlowski (Chuck Drew) and Rachael Kozlowski (Ricky Mueller); his grandchildren, Zoe Andrews and Zaiden Kozlowski; and his siblings, Tracy Folts (Jeff) and Trisha Kozlowski (Carla Quinn).
