He was predeceased by: his parents, Ferg Cooper and Bertha Cooper. He is survived by: his former wife Betty Lou Harrel; his wife Laura Forgey; and his children, Betty, Jack Jr. (Karen), Mike (Chrystal), Kathy Kagey (Dale), Beth Steeves (David), Laura, Lim Forgey Jr., Cathy Chastain (Rick), Lou Lee (Doug) and Lenora McBride (Karl). He is also survived by numerous grandchildren, great grandchildren and two great great grand children.
