He is survived by: his wife Dixie Fazio; his brother Jim Fazio; his daughter Nicole Fazio; his grandchildren, Jordann, Gavin, Talia and Stratton Fazio; his great grandchildren, Garrett, Brody, Kory and Kamryn Fazio; his daughter-in-law Camille Fazio; and his nieces and nephews, Susan, Diane, Lori, Kelly, Brenda, David, Jeff, Bobbie, Brian, Markie, Bradley and Brent.
