He is survived by: his parents, Keith Smith and Donna Smith; his wife Lydia; his children, Zac (Haley), Courtney Jakubowski (Chad) and Logan (Bonita); his granddaughters, Summer Smith and Zadie Jakubowski; and his brothers, Kirk (Leesa), Jeff (Kathleen), Brad (Irene) and Cory (Lisa).
