She was predeceased by: her husband Gilbert; and her siblings, Howard Cook, Beulah Zwart and Velma Fransen. She is survived by: her children, Ken (Linda), Keith (Tami), Kevin (Wanda), Kathy Nienhuis (Eldred) and Patty Syreen. She is also survived by 13 grand-children and 17 great-grand-children.

Phyllis' family requests memorial donations be made on her behalf to W.A.I.F.
