He was predeceased by: his parents, Norman Forsythe Sr. and Lena Forsythe; and his siblings, Joseph, Johnny and Darlene.
He is survived by: his children, Zoeann Forsythe and Jacob Forsythe; and his significant other Konnie Jensen. He is also survived by 7 grandchildren.
