He was predeceased by: his parents, Doyle Sly and Alta Sly; and his brothers, Oren, Ned and Bobby Sly.
He is survived by: his wife Ellen Waddell; his children, Laurrel (Joe) of Maryland, Michelle of Utah and Kurtis (Dana) of Texas; and his grandchildren, Ryan, Ty, Derrek, Kenzie, Amelia, Trent, Brock, Hailey and Beckett.
