He was predeceased by: his parents, Archie Forrest Eastham and Mildred Eastham (McInnis); his sons, Alan and Tim; and his sister Phyllis Mines.

He is survived by: his wife Jane Eastham of Ellensburg; his children, Tony Eastham (Patricia) of Covington and Jana Manterola (Frank) of Ellensburg; his brother Jim Eastham (Patty) of Richland; his grandchildren, Paige Eastham, Jeff Eastham (Karissa), Kelcey Manterola (Dave) and Courtney Manterola (Nick); his great grandchildren, Breelyn, Zoey and Kylee; and his daughter-in-law Ellen Eastham. He is also survived by numerous nieces, nephews, cousins and friends.

Memorial contribution in Forrest's honor are suggested to KVH Hospice, 1506 E. Radio Road in Ellensburg.
