He is survived by: his wife Mary Patricia Cook (Johnson); his children, Cynthia Carlsson (Cook), Randolph Cook, Nicholas Cook and Chelsea Jennings (Cook); and his sister Betty York of La Center. He is also survived by 7 grandchildren.
