He was predeceased by: his parents, Henry Kuiken and Jennie Kuiken (Vander Molen); and his siblings, Sadie Vander Kooi (Lambert), Robert (Dottie), Alice Coplin (Ben), Dickie, Sylvia, Dorothy Assink (Gerald) and Joanna Barker (Don).
He is survived by: his mother-in-law Donna Bame; his brothers, Gerrit, Dick (Judy) and Gene (Sylvia); his granddaughters, Ashley and Ava; and also, Kandi, Jenny and Joshua Ausfahl. He is also survived by numerous nephews and nieces.
